Process and Requirements

Please note: The FAD directory is for patient-facing clinical providers. If you are a resident or are not billable for services, you are NOT eligible to be listed in the FAD directory.

How to be added to the FAD directory

  1. Are you "billable for services"?
  2. Do you provide patient-facing clinical care (i.e. do you interact with patients)?
  3. Are you NOT a resident?
  4. If yes to all the above, please complete the next two steps:
    1. Submit the .
    2. Sign up and attend an upcoming . New official photos are required for all new FAD profiles. ID photos cannot be used.

    How to edit, update, or remove a FAD profile

    • Clinic Directors: If a provider leaves 开云全站app下载, email their full name, netid, and termination date to [email protected].
    • Providers or Clinic Directors: Send FAD edits to [email protected]. Please use the email template below – with the provider’s full name and netid – to describe your request.

      "Dr. Jane Doe (netid) is list as Specialty XYZ, but her specialty is Specialty ABC."
