Please note: The FAD directory is for patient-facing clinical providers. If you are a resident or are not billable for services, you are NOT eligible to be listed in the FAD directory.
How to be added to the FAD directory
- Are you "billable for services"?
- Do you provide patient-facing clinical care (i.e. do you interact with patients)?
- Are you NOT a resident?
- If yes to all the above, please complete the next two steps:

- Sign up and attend an upcoming . New official photos are required for all new FAD profiles. ID photos cannot be used.
How to edit, update, or remove a FAD profile
- Clinic Directors: If a provider leaves 开云全站app下载, email their full name, netid, and termination date to [email protected].
- Providers or Clinic Directors: Send FAD edits to [email protected]. Please use the email template below – with the provider’s full name and netid – to describe your request.
