If you believe you have been exposed to someone with COVID-19, or you are presenting the symptoms of COVID-19, please click on the below link that best describes you.

Choose "Intake' from the dropdown list and click "Record Now" to begin completing the Intake Questionnaire

1. Employees of the University of Illinois at Chicago
   • , then click the blue login button, and login with your University of Illinois System ID and Password
   • When you are in your personal record, choose " COVID Intake" from the dropdown list and click "Record Now" to begin completing the Intake Questionnaire. Click save when complete 
   • UHS will contact you for next steps

     Provide the following information in the body of the email:
     • Name:
     • UIN or Date of Birth:
     • Phone number:
     • Email address:
     • Reason for email: choose one of the following- symptoms, exposure, or symptoms/exposure
